I certify that I am at least 21 years of age and meet all eligibility criteria for the Dance for a Cause bachelor fundraiser application.

I grant full permission to capture my performance via photo and video and the commercial usage of my likeness for charitable promotional missions.

Proceeding constitutes legal acceptance of our Event Terms, Refund Policy, and Privacy Protocols. All applicants undergo mandatory screening.
